Thought that too. I figured it was the bullet he was shot with when he died.

Nope, in this story by Mark Gruewald, Cap was transformed into a woman. :-D

Was he completely transformed into a woman? I thought he had escaped before anything happened.

Mark Gruenwald really put Captain America through the Silver Age story line ringer near the end of his run: de-aging him, turning him into a woman, and the Were-Cap story.

Both he and Paladin were still really muscular, but both were drawn with breasts for a good part of the rest of the story if I recall correctly. So, I'd say that they were at least partially transformed.
