I'm gonna rant a bit, because it makes no sense, how he's kept his job for this long.
For instance: In 2005, you clearly have decided that there's not much more that you can do with Wally West as the Flash. You've had big guns like Mark Waid and Geoff Johns do definitive runs, you've married him off and given him a couple of kids. So you clearly, CLEARLY, make the editorial decision to move Wally West aside in Infinite Crisis, much like you did his predecessor in COIE 20 years before that.
Except...YOU DIDN'T HAVE A FUCKING REPLACEMENT FLASH CREATED AND READY TO GO. Soooooo...you do one of the creepiest things I've ever seen in decades of reading: AGE A KID INTO AN ADULT WHILE LEAVING ALL OF HIS PEERS THE AGES THEY WERE, and tell really weird/bad stories with the character, resulting in the book Flash: Fastest Man Alive going through two creator changes in 13 issues before failing miserably AND YOU KILLING OFF THE KID YOU AGED WHICH WAS CREEPY AS FUCK IN THE FIRST PLACE.
Now think about that for a second. Seriously. Think about that for one second.
Now, I'm not going to sit here and pretend that the Flash is going to move, oh, 85,000 units a month. Hell, I don't know what a good/great Flash book moves on a monthly basis.
But how in the hell does somebody's head not roll for you not having a Flash book on the market that is sustainable -- and if anybody thinks that Flash: Fastest Man Alive was sustainable, feel free to go back up and re-read what I just typed.
Again: They took a kid about 12-14 years old, aged him into his 20s, and left all of his peers the same age as they were -- and had him enter into a serious relationship, to boot.
Whoever authorized that needed to be relieved of duty IMMEDIATELY. And when the book tanked and you had to bring Wally West back because, again, YOU DIDN'T HAVE A VIABLE FLASH READY TO REPLACE THE OLD ONE, somebody ELSE should've been let go.
I can go on. I've got about 50 of these, where I've just sat there and wondered, "Does anybody above Dan DiDio understand how much money they're leaving on the table by not selling books at a level that they could/should be selling?"

-The rape and killing of Sue Dibney
-The JLA mindwiping Batman
-The killing of Lian Harper
-Replacing Jean Paul Valley with a boring character and then bringing him back as a Black Lantern zombie
-Getting rid of Cassandra Cain as Batgirl and replacing her with another boring character
-Turning Geo Force into a douchebag
-The Superboy Prime retcon punches
-Bringing back Barry Allen for no real purpose
-Retconning Superboy's origin and making him a half-clone of Lex Luthor
-Making Barbara Gordon Batgirl again
-Killing off Tim Drake's dad
-Ruining the Outsiders, OMAC and Challengers Of The Unknown books by attempting to write them
-Turning Geo-Force's wife into an evil mutant serial killer
