Thought that too. I figured it was the bullet he was shot with when he died.

Mark Gruenwald really put Captain America through the Silver Age story line ringer near the end of his run: de-aging him, turning him into a woman, and the Were-Cap story.

Both he and Paladin were still really muscular, but both were drawn with breasts for a good part of the rest of the story if I recall correctly. So, I'd say that they were at least partially transformed.
